Health Care Rights and Responsibilities
Our service agreement and HIPAA Privacy Notice Forms provide important information concerning our client’s and caregiver’s rights and responsibilities. Also, all State and Federal laws and policies regarding clients and caregivers will be adhered to.
Complaints or concerns may be directed to the staff member’s direct supervisor. If our patients and families are not satisfied with the response, you may contact the Citywide Home Care LLC Director. The staff person or office personnel will assist you with contacting the appropriate party.
