Home Health:

At times, families are faced with the need for skilled nursing home care or home therapy due to a fall, an injury, or a medical condition. Perhaps you or your loved one was recently discharged from the hospital or your Primary Care Physician believes you may benefit from medical care at home. Citywide Home Care will coordinate your plan of care with the hospital, skilled nursing facility or your designated medical representative to make the transition from facility to home smooth and stress free. Our team of medical professionals, including nurses, therapists, home health aides and coordinators all work together to provide the continuity of care that you deserve.

Citywide Housing Assistance Program:

The Housing assistance program is provided to qualifying individuals or families so that they can afford rent in a decent home and neighborhood. To receive this assistance, applicants must complete a thorough application process and be deemed eligible by Citywide home care. Transportation Program:

Our Transportation program is provided to our participants at no additional cost to the recipient. The service is provided to help patients who lack transportation and cannot afford transportation. Patients can also travel in privacy and usually travel with their caregiver to go shopping, to medical appointments, beauty salons etc. If an emergency were to occur during the commute, our drivers are train in providing CPR and could provide basic medical care. Our vehicles have plenty of space to accommodate large equipment. For example, if someone uses an oxygen tank or uses a wheelchair, they will not have to worry about fitting into the vehicle.

Social Services:

Our compassionate social service case coordinators meet will meet with patient one-on-one to work through whatever issues they are facing. From navigating a crisis to simply answering questions, our case coordinators are here to be good neighbors to anyone in need. Information, referrals, and advocacy are available for a multitude of resources, including (but not limited to) food, clothing, emergency shelter, housing, substance abuse, children’s resources, senior programs, public assistance, and more.

Post-Surgical Assistance:

Recovering from home after surgery can limit you with your daily activities. Whether you are recovering from major surgery, childbirth, or plastic surgery, our compassionate caregivers can assist you to a healthy and complete recovery.

Physical Disability Support:

Arthritis, physical disabilities, and age-related conditions are some of the few circumstances that prevent patients from caring for themselves. Our caregivers can assist you with daily tasks such as mobility, eating, exercising, and grooming.

Care for Dementia Patients:

Our experienced and highly trained caregivers provide the highest quality of care to both patients and their families. Our goal is to help individuals and their loved ones live happy and fulfilling lives.
